1.4    "Sec. 17. EMERGENCY MEDICAL ASSISTANCE DEMONSTRATION
1.5PROJECT.
1.6(a) The commissioner of human services shall implement, beginning January 1,
1.72014, a pilot program to provide alternative services to high-risk individuals with complex
1.8and chronic conditions eligible for emergency medical assistance under Minnesota
1.9Statutes, section 256B.06, subdivision 4. The program must be offered to eligible persons
1.10with two or more chronic conditions, who have had two or more acute care admissions in
1.11the past 12 months.
1.12(b) The pilot program must be designed to provide health care services to eligible
1.13persons in their own environment, to significantly reduce non-emergency inpatient
1.14hospital admissions and long-term nursing facility stays. The program must include visits
1.15by health care professionals to the residences of eligible persons, regardless of whether
1.16the residence is a home, shelter, long-term care facility, residential program, or other
1.17location. The program may include features such as patient education, assistance with
1.18treatment plan compliance, medication management, and coordination of care with other
1.19health care and social service providers.
1.20(c) The commissioner shall select, by September 1, one or more vendors: (1)
1.21experienced with providing in-home primary and acute care to a population similar to
1.22persons receiving emergency medical assistance services; and (2) that have data analysis
1.23skills and modeling capability to identify the target population and evaluate the quality
1.24and cost-effectiveness of the intervention. The vendor contract must guarantee a savings
1.25to the state through operation of the pilot program. Savings generated through this
1.26pilot project must be shared with the vendor. Prior to entering into a final contract, the
2.1commissioner shall require prospective vendors to conduct preliminary analysis and test
2.2model feasibility and effectiveness for the population to be served."
