1.3    "Sec. ... [62Q.676] MEDICATION THERAPY MANAGEMENT.
1.4    A pharmacy benefit manager that provides prescription drug services must provide
1.5medication therapy management services for enrollees taking four or more prescriptions to
1.6treat or prevent two or more chronic medical conditions. For purposes of this subdivision,
1.7"medication therapy management" means the provision of the following pharmaceutical
1.8care services by a Minnesota licensed pharmacist to optimize the therapeutic outcomes of
1.9the patient's medications:
1.10    (1) performing a comprehensive medication review to identify, resolve, and prevent
1.11medication-related problems, including adverse drug events;
1.12    (2) communicating essential information to the patient's other primary care
1.13providers; and
1.14    (3) providing verbal education and training designed to enhance patient
1.15understanding and appropriate use of the patient's medications.
1.16    Nothing in this section shall be construed to expand or modify the scope of practice
1.17of the pharmacist as defined in section 151.01, subdivision 27."
