1.3Page 10, after line 1, insert:

1.4    "Section 1. Minnesota Statutes 2006, section 518.10, is amended to read:
1.5518.10 REQUISITES OF PETITION.
1.6    Subdivision 1. Petition. The petition for dissolution of marriage or legal separation
1.7shall state and allege:
1.8    (a) the name, and address, and, in circumstances in which child support or spousal
1.9maintenance will be addressed, Social Security number of the petitioner and any prior or
1.10other name used by the petitioner;
1.11    (b) the name and, if known, the address and, in circumstances in which child support
1.12or spousal maintenance will be addressed, Social Security number of the respondent and
1.13any prior or other name used by the respondent and known to the petitioner;
1.14    (c) the place and date of the marriage of the parties;
1.15    (d) in the case of a petition for dissolution, that either the petitioner or the respondent
1.16or both:
1.17    (1) has resided in this state for not less than 180 days immediately preceding the
1.18commencement of the proceeding, or
1.19    (2) has been a member of the armed services and has been stationed in this state for
1.20not less than 180 days immediately preceding the commencement of the proceeding, or
1.21    (3) has been a domiciliary of this state for not less than 180 days immediately
1.22preceding the commencement of the proceeding;
1.23    (e) the name at the time of the petition and any prior or other name, Social Security
1.24number, age, and date of birth of each living minor or dependent child of the parties born
1.25before the marriage or born or adopted during the marriage and a reference to, and the
1.26expected date of birth of, a child of the parties conceived during the marriage but not born;
2.1    (f) whether or not a separate proceeding for dissolution, legal separation, or custody
2.2is pending in a court in this state or elsewhere;
2.3    (g) in the case of a petition for dissolution, that there has been an irretrievable
2.4breakdown of the marriage relationship;
2.5    (h) in the case of a petition for legal separation, that there is a need for a decree
2.6of legal separation;
2.7    (i) any temporary or permanent maintenance, child support, child custody,
2.8disposition of property, attorneys' fees, costs and disbursements applied for without setting
2.9forth the amounts; and
2.10    (j) whether an order for protection under chapter 518B or a similar law of another
2.11state that governs the parties or a party and a minor child of the parties is in effect and, if
2.12so, the district court or similar jurisdiction in which it was entered.
2.13    The petition shall be verified by the petitioner or petitioners, and its allegations
2.14established by competent evidence.
2.15    Subd. 2. Social Security number document. In proceedings where child support
2.16or spousal maintenance issues will be addressed, the petition under subdivision 1 must
2.17be accompanied by a separate document that contains the Social Security numbers of the
2.18petitioner and the respondent. The Social Security number document must be maintained
2.19in a portion of the court file or records that are not accessible to the general public."
